Recaptcha and form not working

Hi There

All my forms aren’t working. Using latest versions of DHVC and VC.
See attachment that show Recaptcha error message.

Also, if I remove recaptcha, form is still not working and I get error message asking to insert First Name. (this is supposed to be register form).
