1- I check your site and i see that problem because your theme not support it for widget sitebar in WPBakery, your theme is remove plus(+) in widget title called in WPBakery with javascript. If you want like default layout of your theme, why you need use custom template ?
3- I check again i see that problem because duplicate DHWC Single Product Image on your template (see attachment) Please remove one DHWC Single Product Image and check again.
Also, I see you created private section for mobile. The WPBakery is support responsive, why you need use private style for mobile ?

Dear, the DHVC Form use javascript to submit form with ajax, I check your site and i see the Essential Grid use javascript to open form with lightbox via ajax load.
So, the script of DHVC Form can’t init after Essential Grid load ajax and I don’t know setting add custom javascript after lightbox is open. You can ask to Essential Grid plugin author where is postion can add javascript event callback after open lightbox with ajax ?
