Dear, please download and extract file script.zip in attachment and copy file script.js override to file ‘wp-content/plugins/dhvc-form/assets/js/script.js’ on your site. then please go to edit ‘Conditional Logic’ of Select and remove all action hide. (Conditional Logic like attachment ).

Dear, after create CUSTOM template ‘a-test-product-page’ you need go to edit a WOO Product and select custom template in ‘Page’ meta box(see attachment) . After select page for Woo product, you can view this product on front-end, you will see the product will display with custom template.
