Dear, i have login to your site and add custom code in line 902 to fix your problem,
Code:

if(!isset($image['thumbnail']))
			$image['thumbnail'] = '';

Also, i see product display with image 150×150 (please see attachment).

Dear @controlplusf i have check plugin on my localhost, the thumbnail resize functions still working, i think your problem because wrong in short code setting, please kindly give for me admin account via private reply, i will check detail to help you.

Sorry, i don’t understand your problem, because i have check your site and submit your form, i see your form saved form submit to Database in menu http://www.orionconcepts.net/site/wp-admin/admin.php?page=dhvc-form-entry

Please kindly tell for me more detail your problem ?
