MailChimp 'additional fields' not coming through

Hi,

I have created a form to integrate with MailChimp, at http://www.schoolhouse.edu.au/?page_id=3208

Email, first and last names are sent through to MailChimp, but the final field (SCHOOL), which is also a field in the MailChimp list does not come through.

What do I need to get additional fields like this one to come through into the MailChimp list?
