Conditional Logic help please

Hi,
Please can you help with setting up conditional logic.
If you look at http://dev.macandwood.co.uk/ the form at the bottom.
I have setup conditional logic fields but they do not show on the live site.

The form I have done this is [dhvc_form id=”3132″]

I am trying to make conditional logic work between two Form Select forms in the 3rd row of the form.

On the left Form Select forms “item_materials”

If “Reclaimed Wood” is selected this should show the Select Form on the right “item_finish_reclaimed”
If “Copper” is selected this should show the Select Form on the right “item_finish_copper”

I really do not understand why these two forms on the live form do not show, and how to make this work.

I thought I had set this up correctly.
