No icons in back end

Hi, I mentioned this on the comments at Envato, but icons do not appear in my installation of the back end when creating new forms.
Therefore, it’s nearly impossible to choose the desired icon since only names appear.

I’ve tried installing on 3 WordPress sites to see if it’s a clash with a template, but even on the most basic of WordPress installations it fails for me.

I can send you a login & password by PM on request. Please advise
