Perhaps the line number is different – I just opened in Dreamweaver
I’ve put it back how it was and attached the shortcode.php, so perhaps you could edit and send back?

Hi – I’ve attached a graphic to help explain – When the user fills in the form the fields remain in place – is there a way to have it that once the user has filled in their Username and Password the fields a removed.
Otherwise, the user will think they still have to fill them in
Something like “you are successfully logged in”?
I can’t use another page, as there are several different login pages
