In capital letters are MAILCHIMP fields, in lowercase are DHVC Form fields

EMAIL = user_mail
NOMBRES = nombres
APELLIDOS = apellidos
TRATAMIENT = tratamient
CIUDAD = ciudad
FECHA = fecha
EMPRESA = empresa
TELEFONO = telefono
PAIS = pais
DIRECCION = direccion
CARGO = cargo
HOBBY = hobby
TEMAS = temas
USUARIO= user_login
