1. how can I put each radio button option on a new line?
2. Is it possible to show and hide extra fields after a certain radio? For example when I select radio 2 from the four, extra options will appear below radio 2 not below all of them.

here we could check the form the check box aligned to left and if you select any one or the submit button you we’ll see the problem.
