Can you help me check the compatibility of DHVC Form Plugin with your theme and the plugins you are using.
1. Check plugin: You can deactivate all plugins except 2 visual composer plugins and dhvc form to check.
2. Check theme: You active a default WordPress theme and check again.
3. Check DHVC plugin: You can remove the DHVC form plugin and install again.
