Next time, To fix your problem, you need to create custom default template of the plugin. please download and extract file ‘archive-product.zip’ then download file ‘archive-product.php’ and paste to folder ‘{yourtheme}/dhvc-woocommerce/’ (create folder ‘dhvc-woocommerce’ if not exits).

The first: Field name is required. Please enter single word, no spaces, no start with number. Underscores(_) allowed

The second: form radio2 is on a 1/2 column and the plugin bugs out.
