OK I managed to get it to work. The theme does not ask for first and last name and the controller.php handles the event of no first, last input with an empty string and so it was throwing an error in the json response (invalid merged fields) but the actual error was 404 because of invalid json…had to hard code placeholder first and last names to get it to work.
