Unable to use Visual Composer to create a form! VC Tools not showing

Hi,

After installed the plugin I started to create a new form but the Visual Composer tools are not shown in the New Form page. The Visual Composer tools are completely missing as shown in my screenshot. Please suggest a fix. Thanks!
