Hello! Thank you for your excellent plugin. I’m using it for several years. I suggest you to add:
1) Add attributes for a submit button (on-click event). It would be helpful for configuring goals in Yandex Metrica counter. https://yandex.ru/support/metrika/reports/
2) Add one more way of notification by messengers such as Telegram or WhatsApp.
