Not working in localhost

Hi Support,

I just bought form for visual composer and start to work on it. But when i created form and when i am saving form it takes too much time for reading and after few minuts only white page showing. Even form is not showing on preview.

You can see the attachment for form and settings i used.

Kindly help me quickly.
