Hi,
I have set my module as a Request Information with a total of 9 fields. I have 2 text field, two calendar/dates fields, two multiple choice fields and another text field, a radio button and a captcha, together with a “send” button.
When I go on my website I cannot see the last text field, the radio button or the captcha.
The module is inserted in my homepage with the shortcode.
Could you plese help me?
