Issues with DHVC Form

Hello,

IM having a few issues with my site when DHVC Form is enabled. For one, a yelp sprite social icon goes missing. Second, the 404 page displays ‘contact page form’ in the header (when it should say nothing) when enabled.

Any ideas? Attached are screenshots.
