Form Text Settings: "Label" and "Name"

I’ve purchased 9 licenses for DHVC and really like it, but I think I’ve run into a problem with a long form I’m trying to build at the following link. The form is only about 1/2 completed and I have about 60 more fields to add.

From how I understand the form settings, I have to have unique names for both the “Label” and “Name” for text fields. The problem is the form has several name, phone and address fields. I started by differentiating the labels and names with “my” or “spouse” like the following:

My Phone
my_phone

Spouse Phone
spouse_phone

I thought this was going to work, but my client doesn’t want the form to look like:

My First
My Middle
My Last
My Address
My City
My State
My Zip
My Country

Is there a work around for this? Maybe I’m not doing it right?

I’m going to post a private comment immediately following this post with login information to the WordPress install. If you wouldn’t mind, would you please login and fix the top 2 sections of the form (My Full Name) and (My Spouse’s Full Name) if there is a solution. Then I could see what you did.
