Form submission comes through blank

Hello,

When I filled out my form and sent it as a test, the variables do not show up in the email. Instead it looks like this:

First name:
Last name:
Email address:
Phone number:
Select child’s birth month:
Select child’s birth year:
Select child’s time of day:

How do I fix this?
