Form Submission button working intermittently only

When submitting forms created with DHVC Forms the submit button sometimes works and the form is submitted, other times when submit is clicked the form contents are just cleared (fields made empty) and no submission takes place.

We have made sure that we are using the latest:
WP version
Bridge theme version
WPBakery PAge builder
DHVC form plugin

Please help.
How do I send access to the form to you?
