Form issues with fields and email

Hi,
I have already written to you for help, but I will post it again as instructed.

First, I do not get an email with the data I put inside the module. The website needs to be hosted with Host Switch plus for Chrome (so I will write you the server in a private message).

Also I set my module with various fields but some are not shown in frontend.

Could you please help me? Thanks
