Form has stopped working. Throwing an mysqli error

Hi there

I’ve been using the form builder and all has been well.

I modified my forms so that all the info asked for is added to the email and stored int he server. This now works, but the subsequent ‘Thankyou for submitting page’ now throws this error, and I cannot see how to fix it.
