Form generating error codes on saving and when using

I am using DVHC with VC 4.9.1 and the latest version of the Total theme. When I saved a new form I got the following error message:

This is all that shows on the webpage I can use the back button and see that the work is saved.  However, when someone fills out the form they get the same screen and all I see in the entries is the added comments not the other fields I set.
