There is a bug with the data that is being entered into the database and also emailed. I have a real estate closing form that has multiple inputs with the same label (First Name, Last Name, Address etc.) and all have unique input names, but only the last field with the same label gets recorded in the entry and emailed. I gave all the inputs unique labels and the problem went away. It appears that somewhere the labels are being used when the name should be.
Having unique labels for each field makes for a undesired look:
