DHVC Plugin does not operate after install

I installed the plugin successfully, and navigated to the ‘Form’ button on the dashboard.

I select the ‘Form’ button and land on the new form page, but I can only create a new title for the form-and nothing more. There is no Visual composer icon button visible or available-only the ‘add media’ button.

Please reply with a solution as soon as possible so that I do not have to ask for a refund? Thanks
