DHVC Form causes error 503 on Woocommerce My Account Page

Hello
I hope you are fine.

On my under construction site:https://jewelryaccessories4u.com/
when plugin: DHVC Form is activated i got error: 503 on page: https://jewelryaccessories4u.com/my-account/.

This page works fine only when i am logged in.

I have tryied to disable all other plugins and this error is only when DHVC Forms plugin is enabled.

I need both woocommerce and DHVC Forms.

Please advice.
