Conditional Logic with IMPREZA Theme

Hello,

I’ve purchased your dhvc form plugin (Visual WordPress Form) to use it on the IMPREZA theme:

Problem:
I am trying to build a complex form with a lot of conditional logic, however it doesn’t seem to work as shown here: http://sitesao.com/support/forums/search/conditional+logic/

Example:
I have 4 radio buttons. If radio button 1 is clicked on, a Form Checkbox must appear. However, if radio button 2, 3 or 4 is clicked on, the Form Checkbox must disappear and other fields like Form Text, Form Selects, Radio Buttons, etc. must appear.

What do the settings look like and do you have a specific example as written above?
