Conditional logic not working, but no clue why. Seems interference between items

Hi,
I can’t seem to get the conditional logic working. I got one checkbox to show or hide input fields, but when I placed another row of input fields in the form below that then the checkbox logic broke, and any logic in field below that (not included in the checkbox logic) also failed. Now I can’t seem to get anything working anymore.

Please advise:
– does the logic only work for numeric values or also for matching text values?
– when I set a logic to “show” a form field when matching a value, does that mean that it will by default “hide” that value if the criterium is not met?
– Can you have a look at my form to see why the conditions are not working?(I’ll provide credentials via PM if needed)
