Conditional Logic Hiding Entire Form

I have a simple dropdown with four values. If I tell it to “show” a field when a certain value is selected, the entire form is hidden immediately.

If I tell it to “hide” a field when the same value is selected, it them hides the entire form when then value is selected.

What’s going on here?

I’ve reversed the hide/show logic to show the issue right now. Change “HOW DID YOU FIND OUT ABOUT LOUISO TRUCKING?” to “Advertisement” to see the whole form disappear, instead of just the “Name of Publication” field.
