Conditional Logic Help

I am trying to get the conditional logic form to show multiple fields based on what the user selects.

Question: How many children are you bringing?
Choices: 1, 2, 3, 4, 5

When they select 1:

Child 1 Name Child 1 Date of Birth
(text field) (text field)

When they select 3:

Child 1 Name Child 1 Date of Birth
(text field) (text field)

Child 2 Name Child 2 Date of Birth
(text field) (text field)

Child 3 Name Child 3 Date of Birth
(text field) (text field)

etc……
Is this possible with the conditional login?

This is the test page I am currently working on trying to create the form and you can see what happens when you select 1-5 for the number of children.
