Can I have a hidden field in a form and how to add the form to a page?

Hello, I have a few questions

1. Once I have created a form then how do I add it to a page using Visual Composer?
When I click Add element there is no add form option.

2. Can I have a hidden field in a form which is filled in with a dynamic value. For example I want to use the same form on different pages, but I want it to pass a custom field from the page it is on into the hidden field value.
