After updating wordpress and plugins the registration form stopped working

I have made the registration form with dhvc form after updating wordpress to version 4.9 and the plugins to 2.1.1 I stopped working the registration form when filling in the changes and pressing send refreshes the page and does nothing else.

I hope your prompt help has already made several tests and changes and does not achieve positive results.
