I have built a new form and it seems like it doesn’t work properly if you click on any check box and the form aligned to the left even though it’s supposed to align to right cause your plugin support an RTl website.  here are the link where you could find the problem and please kindly find the attached screenshot which show you where you have to click to show you the form :
