I want to use your plugin with HHVM from facebook and the frontend already works.
This requires a very strict PHP syntax, without any statements, which will then a php warning.

But I get this two errors in backend:

Switch statements may only contain one default: clause in …/wp-content/plugins/dhvc-form/includes/admin.php on line 1389

Switch statements may only contain one default: clause in …/wp-content/plugins/dhvc-form/includes/admin.php on line 1417

Is there any chance you will remove this little problem with the next update? You only need to delete one of the default statements.
