I am experiencing issues where the form does not show up. I am using the shortcode on the page to display the form but nothing is visible. If you look at the source code you can see the form but it’s just not visible. I’ve tried re-creating the form and it seems like there is a limit to the number of fields you can choose before it stops being visible.

Here is what i’ve tried:
1. Tried changing the themes
2. I upgraded the plugin to the latest version
3. WordPress has been updated to the latest version as of 8/7/2015
