I made a form but after click on submit button nothing happen! (just something like loading shows on the button for 1-2 second)

Information do not store on entries or even sending through mail, and do not show submit message. i checked all class names (single word, no spaces, no start with number. Underscores(_) allowed) and nothing is not wrong with it.

Can you check this for me, please.
