first of all: great plugin, great job!

I have two issues:

1. Using Form Select, I get two arrows (grey and black) in the drop-down menu, where I suppose there should only be one black arrow. How can this be resolved?

2. In Notifications email settings, I would like to get a notification mail showing the mail address from the email field as sender. This is not possible, even though Sender email type is set to email field and the corresponding email field is selected. Initially, a sender name with a specific email address was in place, then I wanted to switch to using the email field. The sender name stays upon clicking update in WP, even though it was cleared before.
